At your visit!
New Patient Forms
We kindly ask that all new patients to our practice fill out the following new patient forms prior to your appointment. Don’t hesitate to give our offices a call if any questions arise.
We are so excited to welcome you to the Smiles Dental family! If you would like to expedite your initial appointment, we ask that you print off and fill out these forms and bring them with you to the initial appointment. Let us know if you have any difficulty in printing these forms. We would be more than happy to mail them to you or have you come in 30 minutes early to your appointment to fill them out in-office.
Dental Insurance
We accept most dental insurances and our office is currently in network with several insurance plans and when possible, we will maximize any insurance benefits you have. If we are considered “out-of-network” with your dental plan, you can still use your PPO insurance at our office and did you know…MANY preventative services such as your hygiene appointment with the doctor’s exam are covered 100% by most plans. And at each appointment, as a courtesy to you, we’ll file your dental insurance claim for you so you don’t have to! We strive to make sure our patients are maximizing their dental insurance benefits. Please give our offices a call if you have any questions regarding your insurance.
Payment Options
We make every effort to give our patients a wide variety of options to make treatment affordable. We accept cash, checks, Mastercard, Discover, and Visa.
For patients with larger treatment plans or the need for a payment plan, we are proudly paired with Care Credit and Greenstate Credit Union to help break up the payments.
Additionally, we invite our patients without dental insurance to consider joining our Dental Membership Program. This is a great alternative to dental insurances that allows us to provide you truly individualized care.
Your First Appointment
As a new patient to our office, you can expect your first appointment to include a comprehensive exam and oftentimes x-rays. You can also expect to get a cleaning, in the absence of active gum disease. Following the exam, Dr. Timm will review the x-rays and any findings with you. Our team will make sure you have a complete understanding of your treatment needs and will assist you in scheduling any treatment you may need.
